Who Can Refer a Patient?

You can refer someone to the Inpatient Rehabilitation Center if you are a:

  • Case manager
  • Doctor
  • Family member
  • Insurance company

Who Qualifies for Inpatient Rehabilitation?

To be eligible for our inpatient rehabilitation program, a patient must:

  • Have a medically complex condition that requires the 24-hour availability of a physiatrist
  • Require a multidisciplinary team approach, which includes a physiatrist, rehabilitation nurse, social worker, and physical, occupational and/or speech therapist
  • Be able to actively participate in therapy for 3 hours per day, 5 days per week.
  • Make significant functional improvements in a reasonable period of time (The average stay is 10 to 12 days, with shorter or longer stays depending on the individual’s needs and progress)

Admission Consultation

To determine whether our program is the right fit for your patient, we'll meet with prospective patients. Please gather the following patient information:

  • Discharge disposition
  • Lab reports
  • Patient information including demographics, address and insurance
  • Medical history and physical
  • Medication record
  • Nursing notes - wound care
  • Physical, occupational and speech therapy evaluations with notes about the current treatment
  • Physician, nursing and case management notes
  • Radiology reports

How Long Does it Take to be Accepted?

  • If the patient has insurance through Medicare, Tricare and Kaiser Permanente, you'll learn about acceptance within one business day after we've gathered all necessary admission information.
  • If the patient has private insurance, acceptance decisions may take hours or days based on medical status and insurance response times. Once we get preauthorization and the patient is deemed medically stable for transfer, we can admit the patient.

What to Bring

Please pack the following for your loved one's stay at the Inpatient Rehabilitation Center:

  • Five to seven outfits, including undergarments
  • Comfortable, nonskid flat-soled shoes
  • Seasonally appropriate coat or sweater
  • Toiletry items, including makeup, deodorant, hairbrush, etc.

Please leave valuables such as jewelry at home.

After Inpatient Rehabilitation

Upon discharge, a patient in an acute rehabilitation program is expected to return to home or a community setting, such as an assisted living facility, not to a skilled nursing facility. Depend on us to provide the support needed for safe discharge after inpatient rehabilitation.
