Patient navigators are here to guide you through treatment, improve coordination of care, and address your individualized needs:
What does a navigator do?

  • Guide patients through the care system
  • Help with understanding your diagnosis and treatment options
  • Connect patients with support services at VHC Health or in the community
  • Communicate with your healthcare providers
  • Survivorship care planning
  • Facilitate support groups and workshops
  • Patient advocacy
  • Aid in addressing financial concerns

What Does a Navigator Do?

Your patient navigator can help you and your family:

  • Understand your diagnosis and treatment options
  • Find support services at VHC Health or in the community
  • Communicate with your healthcare providers
  • Feel emotionally supported through up to three sessions of counseling
